Leave of Absence Request

If you are unable to make it to squadron for any reason, please fill out this form and send it to let us know of your issue. Please submit these forms as soon as you are aware of an issue that may affect your attendance.

Forms need to be submitted by 17:30 (5.30pm) on Tuesdays/Thursdays to ensure they are received. If not then your absence may not be authorised despite good reason. If there is a last minute issue please call the Squadron (from 19:00 (7pm) onwards) to let the staff know and understand the issue. If there is an issue that won’t require Leave of Absence (LoA) or is to do with a weekend/off squadron activity, please use the ‘general contact form’ on the Contact Us page.

Please Note: Do not try to enter paragraphs, ‘\’ or ‘/’ into any textbook, especially the Reason for Absence box, as your message may not be fully read. Instead just continuously write using only spaces and punctuation.
